What to expect at your initial appointment

Please obtain a referral from your GP and give us a call to schedule an appointment. Without a GP referral, you are unable to receive your Medicare rebate. Dr Forsyth will prioritise patients that need to be seen urgently.

The duration of your consult will vary depending on the complexity of your problem and your individual needs.

What to bring:

• Your GP referral

• Medicare card

• Health fund details

• Pension or heath care card details

• Any imaging you have had including mammogram, ultrasound, MRI, CT scans

• Any pathology or biopsy results

• A list of your medications
